Adam Ford reports that this change breaks booting on da850-evm and
Bartosz Golaszewski agrees that with the impending release we should
revert the change for now. With that noted:

The MMC boot is currently broken on omapl138-lcdk after enabling the
driver model in SPL. The main problem is the driver's bind callback not
being called after probe in SPL (even with the DM_FLAG_PRE_RELOC flag
specified).
While a proper fix is still being worked on, this hacky changeset at
least fixes the MMC boot on this platform by calling mmc_bind()
manually from probe().

struct mmc_config & struct mmc don't need to be exported over platform
data, but can instead be private in the driver.

When used with a device tree, this will extract the card detect
and write protect pins from the device tree and configure them
accordingly. This assumes the GPIO_ACTIVE_LOW/HIGH is supported
by da8xx_gpio.

With CONFIG_BLK becoming a requirement, the Davinci MMC driver
needs to be updated with DM_MMC support. Since SPL is tiny and
many boards do not support DM in SPL, this retains the backwards
compatibility for those boards who need to initialize MMC manually
in SPL.

When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from. So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry. Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.
In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.
This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents. There's also a few places where I found we did not have a tag
and have introduced one.

Now CONFIG_GENERIC_MMC and CONFIG_MMC match for all defconfig.
We do not need two options for the same feature. Deprecate the
former.
This commit was generated with the sed script 's/GENERIC_MMC/MMC/'
and manual fixup of drivers/mmc/Kconfig.

The way that struct mmc was implemented was a bit of a mess;
configuration and internal state all jumbled up in a single structure.
On top of that the way initialization is done with mmc_register leads
to a lot of duplicated code in drivers.
Typically the initialization got something like this in every driver.
struct mmc *mmc = malloc(sizeof(struct mmc));
memset(mmc, 0, sizeof(struct mmc);
/* fill in fields of mmc struct */
/* store private data pointer */
mmc_register(mmc);
By using the new mmc_create call one just passes an mmc config struct
and an optional private data pointer like this:
struct mmc = mmc_create(&cfg, priv);
All in tree drivers have been updated to the new form, and expect
mmc_register to go away before long.
Changes since v1:
* Use calloc instead of manually calling memset.
* Mark mmc_register as deprecated.

Remove the in-structure ops and put them in mmc_ops with
a constant pointer to it.
This makes the mmc structure smaller as well as conserving
code space (in theory).
All in-tree drivers are converted as well; this is done in a
single patch in order to not break git bisect.
Changes since V1:
Fix compilation b0rked issue on omap platforms where OMAP_GPIO was
not set.

Without this additional delay, some eMMC don't negotiate properly bus width
Tested on:
- Toshiba THGBM2G8D8FBAIB
- Toshiba THGBM4G4D1HBAR
- Micron MTFC4GMVEA (the one giving the problem)
- Hynix H26M64002BNR
- SanDisk SDIN5E1-32G

Check for card detect each time an MMC/SD device is initialized. If card
detection is not implemented, this code behaves as before and continues
assuming a card is present. If no card is detected, has_init is reset
for the MMC/SD device (to force initialization next time) and an error
is returned.

Make existing field b_max field in struct mmc unconditional
and use it instead of CONFIG_SYS_MMC_MAX_BLK_COUNT in mmc_bread
and mmc_bwrite.
Initialize b_max to CONFIG_SYS_MMC_MAX_BLK_COUNT in mmc_register
if it has not been initialized by the hw driver.
Initialize b_max to 0 in all callers to mmc_register.

Added support for MMC/SD cards for Davinci. This feature is enabled by
CONFIG_DAVINCI_MMC and is dependant on CONFIG_MMC and CONFIG_GENERIC_MMC
options. This is tested on DM355 and DM365 EVMs with both the available mmc
controllers.
