After the commit 6aae84769a0b ("gadget: f_thor: Fix memory leaks of
usb request and its buffer"), there is hang-up with ctrl-c in some
udc. It is because req of out_ep is freed before out_ep is disabled.
Fix hang-up with ctrl-c by disabling ep before free req of the ep.

During file download, it only uses 32bit variable for file size and
it limits maximum file size less than 4GB. Update to support more
than 4GB file with using two 32bit variables for file size as thor
protocol 5.0.

The thor sender can send filename without null character and it is
used without consideration of overflow. Actually, character array
for filename is assigned with DEFINE_CACHE_ALIGN_BUFFER() and it
is bigger than size of memcpy, so there was no real overflow.
Fix filename overflow for code level integrity.

When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from. So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry. Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.
In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.
This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents. There's also a few places where I found we did not have a tag
and have introduced one.

U-Boot widely uses error() as a bit noisier variant of printf().
This macro causes name conflict with the following line in
include/linux/compiler-gcc.h:
# define __compiletime_error(message) __attribute__((error(message)))
This prevents us from using __compiletime_error(), and makes it
difficult to fully sync BUILD_BUG macros with Linux. (Notice
Linux's BUILD_BUG_ON_MSG is implemented by using compiletime_assert().)
Let's convert error() into now treewide-available pr_err().
Done with the help of Coccinelle, excluing tools/ directory.
The semantic patch I used is as follows:

The console includes a global variable and several functions that are only
used by a small subset of U-Boot files. Before adding more functions, move
the definitions into their own header file.

Allocate request up to THOR_PACKET_SIZE not the ep0->maxpacket
as the descriptors data depend on the number of descriptors
and this 64 bytes were not enough and the buffer might overflow
which results in memalign failures later.

Dont perform reset at the end of thor download
if configured to do reset off.
Reset may not be required in all cases and hence
provided an option to do so.
The case would be to download the images to DDR instead
of flash device.

Since we support multiple dwc3 controllers to be existent at the same
time, in order to handle the interrupts of a particular dwc3 controller
usb_gadget_handle_interrutps should take controller index as an
argument.
Hence the API of usb_gadget_handle_interrupts is modified to take
controller index as an argument and made the corresponding changes to all
the usb_gadget_handle_interrupts calls.

Some pointers in function download_tail() were not checked
before the use. This could possibly cause the data abort.
To avoid this, check if the pointers are not null is added.

CONFIG_SYS_DFU_DATA_BUF_SIZE may be large to allow for FAT/ext layouts
to transfer large files. However, this means that individual write
operations will take a long time. Allow backends to specify a maximum
buffer size, so that each write operation is limited to a smaller data
block. This prevents the DFU protocol from timing out when e.g. writing
to SPI flash. I would guess that NAND might benefit from setting this
value too, but I can't test that.

since ALLOC_CACHE_ALIGN_BUFFER defines a pointer and not a
buffer, the memset with sizeof(rqt) likely does something else
then intended. Since there is a memcpy directly after it with
the full size, drop the memset completely.

Since dfu read/write operations needs to be flushed manually,
writing to filesystem on MMC by thor was broken. MMC raw write
actually is working fine because current dfu_flush() function
writes filesystem only. This commit adds dfu_flush() to f_thor
and now filesystem write is working.
This change was tested on Trats2 board.

In thor's download_tail() function, dfu_get_entity() is called
before each dfu_write() call and the returned entity pointers
are the same. So dfu_get_entity() can be called just once and
this patch changes this.
