functions
board_usb_init()/_cleanup() should be in board files and don't have
a place in the xhci-omap driver. Weak versions for
board_usb_init()/_cleanup() already exist in common/usb.c
(for host mode) and drivers/usb/gadget/g_dnl.c (for gadget mode).
Therefore, remove init and cleanup functions from xhci-omap and
implement them in the board files.

Invoke enable_usb_clocks during board_usb_init and disable_usb_clocks
during board_usb_exit to enable and disable clocks respectively.
Modifications:
* Enable USB clocks in the OMAP version of the function
board_usb_init.
* Disable USB clocks in the OMAP version of the function
board_usb_cleanup.

A weak version of the function board_usb_init is implemented in:
common/usb.c
drivers/usb/host/xhci-omap.c
To fix the double implementations:
* Convert the board_usb_init function in drivers/usb/host/xhci-omap.c
normal (not weak).
* The function board_usb_init in drivers/usb/host/xhci-omap.c calls to
the weak function omap_xhci_board_usb_init.
* Rename board version of the function board_usb_init to
omap_xhci_board_usb_init.
Done only for boards that defines CONFIG_USB_XHCI_OMAP.
To achieve the same flexibility with the function board_usb_cleanup:
* Add a normal (not weak) implementation of the function
board_usb_cleanup in drivers/usb/host/xhci-omap.c
* The function board_usb_cleanup in drivers/usb/host/xhci-omap.c calls
to the weak function omap_xhci_board_usb_cleanup.
* Rename board version of the function board_usb_cleanup to
omap_xhci_board_usb_cleanup.
Done only for boards that defines CONFIG_USB_XHCI_OMAP.

Now, include/linux/errno.h is a wrapper of <asm-generic/errno.h>.
Replace all include directives for <asm-generic/errno.h> with
<linux/errno.h>.
<asm-generic/...> is supposed to be included from <asm/...> when
arch-headers fall back into generic implementation. Generally, they
should not be directly included from .c files.

xhci omap driver has board_usb_init in xhci_hcd_init but doesn't have
the corresponding cleanup function in xhci_hcd_stop.
Fix it here by invoking board_usb_cleanup() in xhci_hcd_stop().

Add the support for the dra7xx xhci usb host.
dra7xx does not contain an EHCI controller so the headers
can be removed from the board file.
The xHCI host on dra7xx is connected to a usb2 phy so need to
add support to enable those clocks.

Moving the usb/phy code from xhci-omap to the usb/phy directory
and moving the associated phy code over to the new file.
Newer TI processors adding xHCI support will have different PHY configurations
so therefore abstracting this code away will prevent messing around with the
xhci-omap file itself.

Recent patches declares board_usb_init function prototype for a new
usb architecture.
Turning on the OMAP_XHCI defines cause a redefinition compiler failure.
So update the board_usb_init to the latest prototype.
